After-School Program

Prior to a student's first day of attendance, all required registration forms must be submitted to the Program Coordinator.   A completed set of forms is required for each child enrolled in the program. Parents will be asked to complete the following:

Enrollment Agreement	Registration Form	Release Form
Policies and Procedures	Medication Release Form	Insurance Waiver
	Emergency Medical Consent
